通过检索国内外“药物重整”相关文献,整理、分析、汇总,阐明药物重整的现状及国内外差异,总结当前国内不足之处,并提出未来发展方向,为国内临床药师实施药物重整提供参考。对比发现药物重整在国外起步早、重视程度高、实施范围广泛,受到了相关机构和法律的保护,在药物重整的对象、提供者、实施方法与形式以及结果考察指标等方面都有较系统的研究和实践。国内药物重整处于起步阶段,少数医院试点实施,重视程度和实施质量都有待改善。我国需要借鉴国外的先进经验和方法,结合我国现有国情,制订相关法规及药物重整指南,鼓励医、药、护多方合作,构建专业的药物重整专业团队,以保证患者安全、合理用药。
Abstract
To provide the situation and the difference of medication reconciliation at home and abroad by reviewing, analyzing and summarizing domestic and foreign literatures related to medication reconciliation. The current defect and the future development direction of medication reconciliation were summarized to provide reference for its implementation in domestic. Compared with domestic region,medication reconciliation in overseas practice is earlier and popularly recognized, widely implemented and protected by relevant agencies and law. The objects,provider, applied METHODS and mode, outcome measures of medication reconciliation in overseas are explored and practiced more. The medication reconciliation of domestic is in the initial stage, only a few hospitals implemented and needed us to pay more attention to improve it. In order to promote its development in domestic, foreign experience and technology should be learned, regulation and guideline of medication reconciliation should be established in according with the condition and medical environment of our country. Meanwhile, a professional pharmacist team should be built to enhance cooperation in medical care personnel and to guarantee safety and efficiency of drug using.
